Horizons Health provides integrated, team based primary health and wellness services.  We are staffed by an experience nurse practitioner and physician assistant trained in family medicine, and provide care to adults and children 12 years of age and older for most services.  Patients with conditions or health needs outside of our scope of services will be directed to other community health care providers.¹

Payment Methods

Horizons Health accepts cash, credit card, and 3rd party health insurance.²

We offer a Self Pay Fee for Service schedule to assist non-CHC insured/uninsured patients the opportunity to receive medical and wellness services at a reduced rate. We also offer low cost laboratory testing (when performed as part of another service) for our patients that would not be able to afford testing otherwise.

In an effort to eliminate barriers for our uninsured consumers to be eligible to receive Charity Care, we have an Enrollment & Eligibility Specialist from Cape Fear Health Net in the primary care clinic to provide assistance.
