Outpatient Treatment
Outpatient Treatment provides comprehensive and confidential assessment, counseling, education, and referral services for substance users and their families. Fees are affordably based on a sliding scale. Medicaid is accepted.
Coastal Horizons offers Medication Assisted Treatment for Opioid Use Disorders.

- Office Based Buprenorphine (Wilmington, Burgaw, and Shallotte)
Monday thru Friday dispensing hours are 5:30 - 10:00am. Priority dosing on those days is during the first hour of dispensing - from 5:30-6:30am Mon-Fri, including holidays.
Saturday and Sunday dispensing hours are from 5:30-10:00am (with NO priority dosing on those days).

Comprehensive Outpatient Treatment (SACOT)
Coastal Horizons offers an enhanced treatment service called SACOT. This service for people ages 18 years old and older with a primary substance use diagnosis. SACOT is a 16 week program consisting of group therapy, individual therapy, family therapy and case management to link clients with community resources. It is offered daily for 4 hours each day in our Pender and Brunswick offices.
Topics addressed in SACOT include relapse prevention planning, mental health issues, wellness, family roles, emotion regulation, and vocational resources, social skills, and communication skills. Transportation incentives are also provided for our SACOT clients in order to assist them with getting to treatment. We are dedicated to addressing any barriers you may have to treatment. Overall, SACOT outcomes show an overall decrease in substance use during SACOT, and 60% of patients who completed SACOT have remained abstinent from substance use.

Horizons Adult Regional Residential Treatment Services (HARRTS)
The HARRTS program was developed as an innovative treatment approach to engage patients after either acute hospitalizations or inpatient treatment. We serve those individuals with primary substance use disorders as well as co-occurring mental health, medical and housing issues – linking them with a local network of recovery homes.
HARRTS services include:
- Substance Use Intensive Outpatient Treatment.
- Individual Counseling & Psychiatric Evaluations / On-going Care.
- Residential placements through local recovery houses in the Wilmington area.
- Case Management for coordinated access to community services such as DSS for food stamps and Medicaid; medical care through our on-site Primary Care / Health services and assistance connecting to Cape Fear Health Net; clothing; Vocational Rehabilitation; prescription assistance. Regular transition planning sessions throughout the program for a smoother transition to community living. Case management also provides access to emergency needs such as food pantries and toiletries.
- Purchase of some psychotropic and other medications, and connection to prescription assistance programs to assure medical and mental health stability.
- Transportation to and from treatment and bus tickets to assist in accessing community resources.
- Enrollment with Cape Fear Community College for Back to Work Boot Camp.
*Primary care services are available for adults and youth.
