PATH - Projects for Assistance in Transition from Homelessness

Projects for Assistance in Transition from Homelessness (PATH) is a homeless outreach program, serving those who live unsheltered and who are living with chronic and persistent mental illness. The PATH Team assists those living in uninhabitable conditions by connecting them to resources and services. PATH staff consist of Peer Support Specialists, Benefit Specialists, and Qualified Mental Health Professionals that engage homeless individuals to promote sustainability and recovery from mental health issues. PATH has a Veterans component and works with the local VA and community partners to link to available, needed services. PATH staff will also work to guide enrolled individuals to obtain disability benefits.
